To ensure each one of our 50 patients receive this ultimate level of care, admission to the MyMD program is strictly limited and may be denied. Applications may
require a review of medical records and health insurance coverage. The annual membership fee is payable upon acceptance to the program.
Admission is at the sole discretion of Dr. Freedman upon completion of a personal interview and receipt of the signed agreement and fee.
